Partner with Care Pathways
Strengthening support through collaboration
We work collaboratively with education, health and community providers to build capability, foster inclusion and support children and families to thrive.
Stronger teams. Inclusive environments. Better support.
We partner with early learning services, kindergartens, schools, allied health professionals and community organisations to strengthen the support available to children and families.
Through consulting, training and collaborative service delivery, we help teams turn allied health knowledge into practical approaches that work in everyday settings.
Who we partner with
-
Early Learning and Kindergarten
We work alongside educators and leadership teams to understand the needs of their setting and develop practical ways to support children’s participation.
Support can include onsite observation, educator consultation, strategies for communication and regulation, adapting routines and environments, parent information sessions and links to individual assessment or therapy when appropriate.
Individual child observation and information sharing require appropriate family consent. Centre-wide consulting and individual therapy should have clearly agreed scopes and funding arrangements.
-
Schools and Education
We collaborate with teachers, families and existing support teams to support classroom participation, communication, transitions and independence.
Consultation can focus on understanding a child’s needs, reviewing classroom routines and environments, developing practical strategies and supporting consistent approaches across home and school.
-
Allied Health and Referrers
We build collaborative referral pathways with allied health professionals, GPs, paediatricians and support coordinators. With client consent, we share relevant findings, clarify recommendations and coordinate communication to support continuity of care.
Provide clear links for professionals referring an individual client and for organisations seeking a broader working relationship.
-
Training and Consulting
We offer practical professional learning and consultation tailored to your team’s priorities. Topics can include child development, communication, sensory needs, regulation, inclusion, transitions and school readiness.
Options can include workshops, educator coaching, parent information sessions and follow-up consultation to support implementation. We discuss the audience, goals, delivery format and scope before providing a proposal.
